Momma Moments Pilot Program will be Mom /Infant Intervention based Program aimed at supporting the healthy development of infants and young children "at risk" due to biological and psychosocial issues. Through an outreach model and centre-based early intervention services, the mother - child targeted will aim to provide support to parents who are experiencing problems with substance abuse and related issues, as well as their young child (ren) who were exposed to prenatal substances, mental health concerns or at risk of foster care. The Momma Moments program has demonstrated reduced apprehensions and improved outcomes by empowering families. A trauma-informed approach breaks down the many intergenerational barriers that families face such as poverty, homelessness, violence, mental health, and addiction.
The Momma Moments Pilot Program would promote the importance of social, emotional, and healthy development for children of the program with a direct service model that supports stability for the mom through various supportive interventions. The goal/objective of this pilot is to enhance the quality of parent-child interactions, follow-up for medical appointments, reduce barriers that families would face in relation to poverty, housing, transportation, financial and coordination of children in care.
There is a fragmentation of medical services for these high-risk families, as they often do not have stable primary care providers. They often do not have stable housing or stable contact information so appointments are missed. A deficit in the continuity of care, gaps in communication, inconsistency of service and a lack of follow- up for developmental clinics can be detrimental to this underserviced population.
This pilot-based initiative is for 6 young mothers aged 16-29, for the St. Johns and surrounding area with the with the duration of this pilot to be 16 weeks. Prior to the completion of the 16 weeks the team working with the family as well as community partners can assess the situation and make recommendations for continuation of supports through a formal referral for the Momma Moments Program or to other community agencies deemed to be an appropriate fit.
